***Please Note that not all doctors are providers for all insurances, and not all locations may be set up to take your insurance. Also, some insurances do not cover routine eye care. Please call our office BEFORE you desire to use your insurance as a means of payment. We will be glad to help you determine what services you are eligible to use in our office.
